on medical application of preparation:
CORDIAMINE (Cordiaminum)

General characteristics: Niketamide (diethylamide of nicotinic acid).
  basic physical-chemical properties: clear colourless or slightly yellowish or greenish liquid;
  composition: 250 mg of niketamide in 1 ml of water for injections.
Production form. 25% niketamide solution in syrette for injections.
Pharmacotherapeutic group. R07A B Respiratory analeptics. R07 A B02 Niketamide.
Pharmacological properties. Analeptic of mixed type effect. Effect mechanism consists of two components: central and peripheric. The first one exert direct influence on vasomotor center of medulla oblongata which results in its stimulation and increasing of system arterial pressure (especially at primary oppression of motor center). Peripheric component deals with stimulation of chemoreceptors of carotid sinus which results in increasing of rate and depth of respiratory movement.
 At intravenous injection of the preparation rhythm becomes more frequent, rate and depth of respiration increase, arterial pressure increases slightly and for a short period of time.
 The preparation does not exert direct stimulating effect on heart and does not have direct stimulating vasoconstrictive effect.
Pharmacokinetics. Not studied.
Indications for application. Collapse, asphyxia, shock state appearing during surgical interventions and postoperative period, acute and chronic circulatory disturbance, oppression of respiration of patients with infectious diseases, narcotic, soporifics and analgetics poisoning.
Mode of application and dosage. Before applying open contour package. Without removing the cap from the needle-extension screw the needle on the syrette against the stop in order to pierce the body and provide free movement of the solution into the needle. Remove the cap from the needle holding the syrette on to the extension. Remove air by slightly pressing the syrette with fingers and without unclenching fingers inject the needle. Squeeze out the content of the syrette and remove the needle without unclenching fingers.
 The preparation is prescribed to adults subcutaneously, intramuscularily or intravenously by 1-2 ml 1-3 times per day. At narcotic, soporifics and analgetics poisoning cordiamine dose is to be increased up to 3-5 ml once.
 The highest doses: single - 2 ml, daily - 6 ml. At narcotic poisoning the highest single dose is - 5 ml.
Side effect. Anxiety, muscular twitching starting from orbicular muscle of the mouth, reddening of the face, pruritus of skin integument, vomiting, breach of cardiac rhythm. Allergic reactions are rare.
Contraindications. Predisposition to spastic reactions.
Interaction with other medicines. Cordiamine strengthens effects of psychostimulants, antidepressants. It decreases effect of narcotic analgetics, hypnotics, neuroleptics, tranquilizers, anticonvulsive agents, favours development of phthivazid intolerance. Against a background of profound narcosis cordiamine does not make an effect.
Overdosage. Tonoclonic spasm is possible.
Peculiarities of application. Injection of cordiamone into vein should be made slowly. Subcutaneous, intramuscular injections of the preparation are painful.
Conditions and period of storage. Store in cool, dark place. Storage period - 1 year.
Terms of distribution. Under prescription.
